Services

Quality Care Community Services, Inc. will provide residential in-home supports for individuals who are developmentally challenged.  These services are designed to empower individuals to acquire, refine, or maintain an optimum level of functional skills necessary to live in a community setting.  Services are offered in both residential and community environments.  Training and assistance are provided in areas of personal care, socialization, communication, community integration, behavior management, health, safety, and daily living skills.  Families will be encouraged to implement training in an effort to promote consistency as the individual is learning.

Individuals participating in the program are encouraged to participate in the community to the fullest extent of their ability and to utilize the various resources available.  This is also an opportunity for each individual to develop community integration skills, socialization skills and to enhance their self-esteem by establishing themselves as a valued member of their community.  A program schedule will be provided for each individual that will include planned community activities of the individual’s choice.  A comprehensive activity and community resource guide will be utilized to supply the individual with a wide variety of activities to choose from.  Quality Care Community Services, Inc. is committed to promoting self-esteem and quality of life for persons with developmental disabilities.  These services are developed to open a wide variety of opportunities that traditionally have not been available to individuals with developmental disabilities because of stereotypes and lack of community inclusion.  Activities for individuals will reflect their choices and personal preferences.

Population Served/Admission Criteria

 

​Persons receiving services from Quality Care Community Services Inc. must meet the following criteria for admission:

 

  1. Must be at least 5 years old.

  2. Must have a primary diagnosis of Intellectual Disabilities as defined in the Code of Virginia 37.1 disabilities, or have other documented needs in two or more adaptive skill areas. 

  3. An individual must have an established need for support and have limitations in adaptive skills as designated by the Level of Functioning (VIDES) assessment in two or more of the following areas:

    • Personal care

    • Communication

    • Socialization

    • Community Involvement

    • Use of community resources

    • Task Learning skills

    • Mobility

    • Health and safety

    • Functional academics

    • Behavior management

  4. Must have a physical exam within 30 days of admission to the program, have a current Tuberculosis, Hepatitis B screening, and be free of communicable diseases.

  5. Must be able to function in a 1:1 staff/individual ratio.

  6. Must have an appointed authorized representative upon admission to the program, if under the age of 18 or incapable of making decisions for themselves.

  7. Must participate to his/her fullest extent with the development of an individual service plan and its implementation

Additional assessment tools such as: SIS (Sensory Intensity Scale) may be utilized to assess individual needs.
